Rules & Guidelines

  • The Wellness Program is strictly for our valued patients that are not currently insured and cannot be used in conjunction with dental insurance.
  • New patients will need to complete a comprehensive exam & X-rays before entering the Wellness Program.
  • Patient will prepay for the recommended Wellness Plan indicated above.
  • The patient will have a hygiene appointment every six (6) months. Periodontal and Ortho patients will be seen every four (4) months for their hygiene appointments.
  • We reserve one hour for you with our hygienist. If for any reason you need to reschedule or cancel your appointment, we ask that you provide 48-hour notice. Please refer to our Cancellation/Broken Appointment Policy in your New Patient paperwork for details.
  • This Wellness Plan Agreement is valid for one (1) year from the date the patient entered into this agreement. (See date & signature below). Any unused visits will be forfeited if not used within the agreement period and are not subject to refund.
  • The patient will be entitled to a 10% discount* off of our office fees for any treatment needed during the Wellness Plan agreement period. (Discount not valid if using CareCredit)
  • This Wellness Plan is only for the patient as indicated in this agreement and is non-transferable to any other patient or dental practice.
